The tickets were sold in the Norwood section of the The Bronx, in Levittown, in Manhattan, in Richmond Hill and in Watertown, at the following locations.
- 3161 Norwood Deli Corp located at 3161 Bainbridge Avenue in The Bronx, which sold one prize-winning ticket worth $14,254.50
- Desai Newsstand located at 100 W57th Street in Manhattan, which sold one prize-winning ticket worth $13,872.00
- Family Grocery & Deli located at 107-27 Atlantic Avenue in Richmond Hill, which sold one prize-winning ticket worth $13,872.00
- Sunoco Gas Station located at 1222 Washington Street in Watertown, which sold one prize-winning ticket worth $13,872.00
- Turnpike Gas Corp located at 3300 Hempstead Turnpike in Levittown which sold one prize-winning ticket worth $13,872.00
Take 5 numbers are drawn from a field of one through 39. Drawings take place every evening at 10:30 p.m. Winning tickets at all prize levels may be cashed up to one year from the date of the drawing.
