The MTA has announced the death of a female transit worker who was only three months into her role at its Mosholu maintenance yard in Bedford Park.

In a brief statement on Thursday, Aug. 11, the agency wrote, “The Transit family is heartbroken to have lost a colleague as she was putting a train into service at the Mosholu Yard to help move New Yorkers, and we are conducting a thorough investigation to determine what happened.”

The agency said the victim, Azure Forde, 38, joined NYC Transit on May 15, 2022, as a train operator.

The news agency reported that personnel had reportedly attempted to perform CPR on the worker, who reportedly made contact with the 3rd rail. Norwood News is attempting to corroborate this information.

The news network also reported that, according to the FDNY, the worker was deceased upon their arrival, and that she had just started working for the transit company.
When contacted about the incident, a spokesperson from the FDNY said they received a report of an unconscious person at 8.28 a.m. on Aug. 11, at Moshulu Parkway / Paul Avenue. “Reports of person unconscious on train tracks,” the spokesperson said, and added that one patient was transported to Montefiore Hospital.

A worker in the area told Norwood News he’d heard about what happened, but that it had happened at the adjacent Mosholu train yard.

When contacted about the incident, the NYPD said police responded to a 911 call at 8.20 a.m. on Aug. 11 regarding an aided person at 3565 Paul Avenue (MTA facility – Mosholu Train Yard).
“Upon arrival, officers observed a 38-year-old female, unconscious and unresponsive lying facing up on the tracks at the above location,” a police spokesperson said. “EMS responded and transported the aided to Montefiore Hospital where she was later pronounced deceased.”
The spokesperson added, “The female did not have trauma indicative of being struck by a train or touching the third rail. The medical examiner will determine the cause of death.”

On the same day, as reported, MTA chief safety and security officer, Patrick Warren, issued a statement further to news that a person was found to have been carrying a gun in the Woodlawn Road Subway Station, serving the 4 line.
“The NYPD’s presence at subway stations continues to pay off as alert officers spotted a woman evading the fare, and when stopping her, discovered a loaded gun and an outstanding warrant,” Warren said. “In addition to taking a weapon out of the transit system, it drives home that those who want to prey on people in the transit system should expect to be caught and face justice.”

The incident happened on Wednesday, Aug. 10, at 6:04 p.m. according to the MTA and the NYPD. The transit bureau Bronx/Queens task force officers were on patrol at the Woodlawn Road station when they observed the woman walk through the emergency gate to avoid payment of fare.
“A field search resulted in the recovery of a silver Springfield Armory 9mm firearm from the perpetrator’s backpack,” officials said. “The firearm had one round in the chamber and 7 rounds in magazine.”
